Medical Examination Truck Licence
A medical examination (health declaration) is mandatory for the truck driving licence. Here you can read exactly what to do.
What is the health declaration?
For driving licence C or CE you must complete a Health Declaration (Eigen Verklaring) and have it assessed by a doctor. The CBR will determine based on this whether you are medically fit to drive a truck.

How does the examination work?
From application to approval in 4 steps.
Request Health Declaration
Download the Health Declaration via Mijn CBR or purchase it at a municipality office. Cost: €42.50.
Visit a doctor
Make an appointment with your GP or a certified medical examiner. Bring the Health Declaration.
Report sent to CBR
The doctor fills in the medical section and sends the report to the CBR.
Receive approval
The CBR reviews your application. Upon approval you can schedule your theory exam.
What gets tested?
The doctor checks the following areas to determine if you are medically fit.
Vision
Eye test for acuity, field of vision and colour perception. Minimum 0.8 (with correction).
Hearing
Check that you can hear sufficiently to function safely in traffic.
Heart & blood vessels
Blood pressure measurement and assessment of the cardiovascular system.
Neurological
Check for conditions that may affect driving ability (epilepsy, etc.).
General health
Assessment of your general condition, weight and medication use.
Diabetes
With diabetes, extra assessment is needed. With good control, the licence is often still possible.
